Youth Event Liability Waiver

This waiver is valid for one year from the date of signature. It covers participation in all OneChurch Aloha youth ministry activities, whether held on church property or offsite. Events may include (but are not limited to) camps, summer activities, trips and outings, seasonal events such as pumpkin patch visits, lock-ins, and other church-sponsored gatherings.

Parent/Guardian Consent & Medical Release 

I, the undersigned, am the parent or legal guardian of the minor child named below and give permission for my child to participate in the above event / overnight youth event organized by OneChurch Aloha, a Foursquare Church. I understand that this event will involve overnight accommodations on church property and may include indoor and outdoor group activities, games, meals, and supervised recreational time.

By providing the following information, I authorize any adult staff or volunteer leader representing OneChurch Aloha to obtain emergency medical treatment for my child should it be deemed necessary. I understand that reasonable effort will be made to contact me prior to treatment. 

Overnight Safety & Conduct Agreement 

• All adult volunteers have passed required background checks and been trained per the Foursquare Child and Youth Protection Manual. 

• There will be a minimum of two screened adult leaders present at all times. 

• Boys and girls will be assigned separate sleeping areas with adult leaders assigned accordingly. 

• My child is expected to abide by the event rules and treat others with respect. Inappropriate behavior may result in early dismissal from the event at the guardian’s expense. 

• Personal electronics may be limited or collected at discretion of leaders to maintain a safe, connected environment. 

Media/Photo Release

I give permission for OneChurch Aloha to photograph and/or record video of my child during the event for use in church publications, promotional materials, or social media. I understand that my child’s name will not be used in connection with these images without additional consent. 

Off-site Travel Clause 

I understand that if any portion of this event involves transportation or activities off church property, I will be notified in advance. By signing below, I acknowledge that I give general permission for my child to be transported by church-approved adults in such cases, with safety and accountability procedures in place. 

Liability Release 

I understand that, as with any activity, there are inherent risks involved, and I agree to release and hold harmless OneChurch Aloha, its staff, volunteers, and the International Church of the Foursquare Gospel from any and all liability for injury, loss, or damage to person or property that may occur during or in connection with this event.
